چکیده

Introduction: Macroscopic hematuria involving bladder varices is a rare presentation to Emergency Department (ED). Its presentation warrants immediate diagnosis and management to prevent severe anaemia and hypovolaemia. Injection therapy may be an option that can be offered as an alternative procedure to treat gross hematuria. It is considered less invasive when compared to an open surgery especially in high morbidity patients. We described an alternative treatment modality in the treatment of bleeding bladder varices using endoscopic injection of Histoacryl® (N-butyl-2cyanoacrylate). Case report: This case report describes a female patient who presented to the ED with bleeding bladder varices secondary to portal hypertension in an ileal augmented bladder. The report discusses an alternative treatment modality to treat bleeding bladder varices using endoscopic injection of Histoacryl® (N-butyl-2cyanoacrylate). Discussion: Macroscopic hematuria is a common presentation to the Emergency Department (ED). Nevertheless, hematuria secondary to urinary bladder varices is an unusual presentation. Bladder varices secondary to portal hypertension is rare since the bladder wall is not the usual collateral route for venous splanchnic blood flow. Bladder varices may occur in patients with portal hypertension in circumstances where the normal splanchnic collaterals fail to develop due to prior obliteration from treatments such as surgery, sclerotherapy or ligation. The second probability is when the anatomy of the venous drainage of the bladder is altered from surgery such as bladder augmentation with a bowel segment. Conclusion: Histoacryl® injection therapy can be regarded as an effective alternative in the management of patient with bleeding bladder varices.

Safe and successful endoscopic initial treatment and long term eradication of gastric varices by EUS-guided Histoacryl (N-butyl-2-cyanoacrylate) injection

OBJECTIVE: Optimal endoscopic treatment of gastric varices is still not standardized nowadays. Actively bleeding varices may prohibit a successful endoscopic injection therapy of Histoacryl® (N-butyl-2-cyanoacrylate). Since 2006, we have treated gastric varices by standardized endoscopic ultrasound (EUS) guided Histoacryl injection therapy without severe adverse events. MATERIAL AND METHODS: We...

Evaluation of therapeutic effects and serious complications following endoscopic obliterative therapy with Histoacryl

BACKGROUND AND AIMS Our aim was to evaluate the utility of endoscopic obliterative therapy with Histoacryl for gastric varices and to investigate the incidence of serious complications. METHODS Endoscopic obliterative therapy with Histoacryl was performed on 129 gastric variceal patients. Sixty-five patients had cardiofornical varices, and 64 had fundal varices. Forty-five of 129 patients wer...

Endoscopic Management of Bleeding Ectopic Varices With Histoacryl

Bleeding from antral and duodenal varices is an uncommon feature in patients with portal hypertension. We report a patient with cirrhosis and portal vein thrombosis, who had a massive bleed from antral and duodenal varices. Bleeding was controlled with endoscopic injection of varices using histoacryl. Endoscopic treatment and the relatively uncommon occurrence of antral and duodenal varices are...

Balloon-Occluded Percutaneous Transhepatic Obliteration of Isolated Vesical Varices Causing Gross Hematuria

Gross hematuria secondary to vesical varices is an unusual presentation. We report such a case recurrent gross hematuria in a male patient who had a history of bladder substitution with ileal segments that had been treated by balloon-occluded percutaneous transhepatic obliteration of vesical varices.

Endoscopic Hemostatic Treatment of Gastric Varices during Pregnancy by Injection of Histoacryl

The occurrence of pregnancy is relatively rare in patients with portal hypertension, and has been reported as clinical cases. However, few studies have evaluated the effectiveness of injecting histoacryl during pregnancy. The choice of treatment for varicose bleeding during pregnancy has been described in the literature based on the elastic ligature. We report the second case, to our knowledge,...
